All broker appointment/contracting applications are located in the licensing forms tab above. Please download, complete and sign applications for all carriers you would like to write business with. You MUST be appointed with each carrier prior to submitting new business. You can either fax (516-719-0876) or email (lsaroopchand@morstan.com) completed applications with your current license (must be a life license for all Life and DI) and copy of your errors and omissions (E & O) coverage. Please note some carriers suggest you do not submit licensing until you write your first line of business with them.
