Check for $80 made payable to "Superintendent of Insurance"

Completed MetLife Broker Appointment application (for sponsorship)
Life Contracting Requirements:

Current Life License

Errors and Omissions (required for most carriers)
Health & Long Term Care Contracting:

Current P & C license
